LPN Team Lead

RESPONSIBILITIES:

Lead the team on your assigned unit, assuring completion of all assigned resident care needs
Provide direct "hands-on" patient care, including passing medications and performing treatments
Oversee and guide CNA's work to ensure quality resident care and completeness
Assists in the admission, transfer & discharge of residents
Communicate closely with the Supervisor/Manager in terms of resident problems, progress, and care plans
Direct and demonstrate individualized supportive care to the residents and their families while working cooperatively as an integral member of the multidisciplinary team

ATTRIBUTES OF SUCCESSFUL LPNs:

Current Licensed Professional Nurse license in the State of New York
Good organizational and communication skills
Respect for the privacy, dignity, and confidentiality of all residents
Prior experience in long-term care is helpful but not necessary
