JOB TITLE: LPN Supervisor
DEPARTMENT: NURSING
SUPERVISOR: RN Program Manager
WORK AREA: Garden House
I. JOB SUMMARY:
Plans and implements the health needs of the residents in the Assisted Living Residence, Enhanced Assisted Living Residence and Special Needs Assisted Living Residence, assuring that each resident receives the necessary services,
Diet, medications, treatments and supervision of HHA’s as prescribed and in accordance with the ISP and physician’s orders
II. ESSENTIAL COMPETENCIES:
1. Understands and Implements and enforces with HHA staff, the policies and ISP for each resident.
2. Assesses the individual needs of the residents to ensure that an adequate level of personal hygiene is maintained, that medications are being taken as prescribed and the physical, mental, social, recreational and religious needs of the residents are being provided for by the programs offered in the facility.
3. Plans and provides for, with the Program Manager, the overall comfort, safety, and well- being of the resident.
4. Assesses all health and emergency concerns and coordinates on-going nursing care.
5. Compiles admission information for the medical chart when Program Manager is not available.
6. Evaluates resident on admission and completes appropriate documentation.
7. Maintains records according to facility policies and New York State Health department regulations and guidelines.
8. Monitors and updates medical records, monthly medication records, & self-medication audits.
9. Maintains and monitors accurate records for narcotic substances prescribed for residents.
10. Receives and processes physician orders and provides notification to pharmacy any necessary medication changes.
11. Assists residents in taking prescribed medications as ordered by the physician. Gives Insulin and B-12 injections, checks blood sugars, Blood Pressures and pulse as ordered.
12. Orders medications weekly and as needed.
13. Prepares Physicians consult sheets for medical appointments. Assists with scheduling
of necessary medical appointments.
14. Coordinates, reviews, and completes shift report with other nursing personnel and paraprofessionals.
15. Obtains appropriate documentation upon re-admission of residents to Garden House
16. Assigns and supervises specific duties of the HHA’s to ensure residents receive adequate assistance with activities of daily living.
17. Participates in the training, orientation, and supervision of HHA staff.
18. Participates in the evaluation, coaching, and disciplinary process for HHA staff.
19. Reports safety concerns and any necessary repairs to maintenance department.
20. Assists in the monitoring and completion of necessary HHA documentation
21. Attends staff meeting, mandatory in-services and professional development programs as required.
III. OTHER RELATED COMPETENCIES:
- May provide, participate in or arrange resident and/or family counseling or supportive care.
- Monitors daily staffing to ensure and assign adequate staff coverage for residents care.
- Maintains supplies and checks weekly and as needed for replenishment.
- Provides on going communication with families, residents, and medical management team to ensure appropriate notifications regarding medical conditions and updates.
- Maintain and complete any necessary paperwork associated with laboratory testing for the residents and emergency care.
- Complete phone report to all facility managerial staff.
- Completion of all paperwork related to on site physician weekly visits.
IV. POSITION SPECIFIC COMPETENCIES:
Employee must be 18 years of age.
Must be empathetic and sensitive toward and able to work with the geriatric population.
Must be able to maintain a leadership role.
V. WORKING CONDITIONS:
High chance of exposure to human bodily fluids.
Need to maintain the privacy, dignity, and rights of the individual residents.
VI: Must be able to speak, read and write English and speak the predominant language
of the residents.
Must be able to lift greater than 30 pounds.
Must be able to walk/stand for extended periods of time.
